How Referrals Work

Submit the Referral

Call, fax, or email us with the patient's information and physician orders. We accept referrals 7 days a week.

Insurance Verification

Our intake team verifies the patient's insurance benefits and eligibility — typically within a few hours.

Care Coordination

We contact the patient and family to schedule the initial visit, often same-day or next-day.

Clinical Updates

You receive timely progress notes, OASIS summaries, and discharge reports to keep your records complete.

What We Need

Required Referral Information

To process your referral quickly, please have the following information ready when you call, fax, or email us.

  • Patient name, date of birth, and contact information
  • Diagnosis (primary and secondary ICD-10 codes)
  • Physician orders / Plan of Care (485)
  • Insurance information (Medicare ID, plan name, member ID)
  • Homebound status documentation
  • Recent hospital discharge summary (if applicable)
  • Referring physician name, NPI, and contact

What We Offer

Home Health Services

All services are provided in the patient's home by licensed, Medicare-certified clinicians under physician orders.

Skilled Nursing

Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) provide clinical care in the home.

  • Wound care and dressing changes
  • Medication management and education
  • IV therapy and injections
  • Disease management (CHF, COPD, diabetes)
  • Post-surgical monitoring
  • Catheter care and ostomy management
  • Patient and caregiver education

Typically 2–5 visits per week based on physician orders

Physical Therapy

Licensed Physical Therapists (PTs) restore mobility, strength, and function after illness, injury, or surgery.

  • Gait training and balance rehabilitation
  • Post-surgical rehab (hip/knee replacement, fractures)
  • Strengthening and range-of-motion exercises
  • Fall prevention programs
  • Pain management techniques
  • Assistive device training (walker, cane)
  • Home safety evaluation

Typically 3–5 visits per week during active rehab phase

Occupational Therapy

Licensed Occupational Therapists (OTs) help patients regain independence in daily living activities.

  • Activities of daily living (ADL) training
  • Bathing, dressing, and grooming retraining
  • Fine motor skill rehabilitation
  • Cognitive rehabilitation strategies
  • Adaptive equipment recommendations
  • Home modification assessments
  • Energy conservation techniques

Typically 2–3 visits per week, coordinated with PT when applicable

Certified Home Health Aides (HHAs) provide personal care and support under the supervision of a skilled nurse.

  • Personal hygiene and grooming assistance
  • Bathing and dressing support
  • Ambulation and transfer assistance
  • Vital signs monitoring
  • Light meal preparation
  • Medication reminders
  • Companionship and emotional support

Available daily or several times per week as ordered by physician

Home Health Aide services must be ordered alongside a skilled service (nursing or therapy).
