Skilled Nursing

Clinical care delivered in the comfort of home

Our Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) bring hospital-level clinical expertise directly to your patient's home. Skilled nursing is the cornerstone of home health — coordinating care, educating patients and families, and managing complex medical conditions with compassion and precision.

What We Provide

  • Wound care and advanced dressing changes
  • Medication management, reconciliation, and patient education
  • IV therapy, injections, and infusion management
  • Chronic disease management — CHF, COPD, diabetes, hypertension
  • Post-surgical monitoring and incision care
  • Catheter care and ostomy management
  • Vital signs monitoring and clinical assessment
  • Lab draws and specimen collection
  • Fall risk assessment and prevention planning
  • Patient and caregiver education and training

Who Qualifies

  • Patients recently discharged from a hospital or skilled nursing facility
  • Patients with new or worsening chronic conditions requiring monitoring
  • Patients requiring wound care, IV therapy, or injections at home
  • Any patient who is homebound and has a physician order for skilled nursing

Visit Frequency

Typically 2–5 visits per week based on physician orders and patient acuity

Medicare Coverage

Covered 100% by Medicare Part A when homebound criteria are met and physician-ordered.

Physical Therapy

Restoring mobility, strength, and independence

Our licensed Physical Therapists (PTs) design individualized rehabilitation programs to help patients recover from surgery, injury, or illness — all in the safety and familiarity of their own home. We focus on restoring function, reducing pain, and preventing future injury.

What We Provide

  • Post-surgical rehabilitation — hip replacement, knee replacement, fractures
  • Gait training and balance rehabilitation
  • Strengthening and range-of-motion exercise programs
  • Fall prevention assessment and training
  • Pain management through therapeutic exercise and manual techniques
  • Assistive device training — walker, cane, wheelchair
  • Neuromuscular re-education after stroke or neurological events
  • Home safety evaluation and hazard identification
  • Stair training and functional mobility
  • Patient and caregiver home exercise program instruction

Who Qualifies

  • Patients recovering from orthopedic surgery (hip, knee, shoulder)
  • Patients with balance disorders or history of falls
  • Stroke or neurological condition patients needing mobility rehab
  • Any homebound patient with a physician order for physical therapy

Visit Frequency

Typically 3–5 visits per week during the active rehabilitation phase

Occupational Therapy

Rebuilding independence in everyday life

Our licensed Occupational Therapists (OTs) help patients regain the ability to perform daily activities — bathing, dressing, cooking, and more — safely and independently. OT bridges the gap between clinical recovery and real-life function, empowering patients to live with greater confidence.

What We Provide

  • Activities of daily living (ADL) retraining — bathing, dressing, grooming
  • Fine motor skill and hand function rehabilitation
  • Cognitive rehabilitation strategies for memory and attention
  • Adaptive equipment assessment and recommendations
  • Home modification assessments for safety and accessibility
  • Energy conservation and work simplification techniques
  • Upper extremity strengthening and coordination
  • Sensory processing and integration therapy
  • Caregiver training for safe patient assistance
  • Return-to-work or return-to-activity planning

Who Qualifies

  • Patients with difficulty performing self-care after surgery or illness
  • Stroke patients needing upper extremity or cognitive rehabilitation
  • Patients with arthritis, Parkinson's, or neurological conditions
  • Any homebound patient with a physician order for occupational therapy

Visit Frequency

Typically 2–3 visits per week, often coordinated with physical therapy

Speech Therapy

Restoring communication and safe swallowing

Our licensed Speech-Language Pathologists (SLPs) evaluate and treat communication disorders, swallowing difficulties, and cognitive-communication impairments in the home setting. Speech therapy is often critical after stroke, neurological events, or head and neck procedures.

What We Provide

  • Swallowing evaluation and dysphagia treatment (modified barium swallow)
  • Aphasia therapy — expressive and receptive language rehabilitation
  • Cognitive-communication therapy for memory, attention, and problem-solving
  • Voice disorders and vocal cord rehabilitation
  • Articulation and speech clarity improvement
  • Augmentative and alternative communication (AAC) device training
  • Caregiver education on safe feeding and communication strategies
  • Post-laryngectomy voice rehabilitation
  • Fluency and stuttering therapy
  • Tracheostomy and ventilator-dependent patient communication support

Who Qualifies

  • Stroke patients with aphasia, dysarthria, or swallowing difficulties
  • Patients with Parkinson's disease affecting speech or swallowing
  • Post-surgical patients (head, neck, or neurological procedures)
  • Any homebound patient with a physician order for speech therapy

Visit Frequency

Typically 2–3 visits per week based on diagnosis and treatment goals

Home Health Aide

Personal care and support with dignity

Our certified Home Health Aides (HHAs) provide compassionate personal care and assistance under the direct supervision of a Registered Nurse. HHA services help patients maintain hygiene, safety, and comfort at home — supporting both the patient and their family caregivers.

What We Provide

  • Personal hygiene assistance — bathing, grooming, oral care
  • Dressing and undressing support
  • Ambulation assistance and safe transfer techniques
  • Vital signs monitoring (temperature, pulse, blood pressure)
  • Light meal preparation and nutrition support
  • Medication reminders (non-administration)
  • Incontinence care and skin integrity monitoring
  • Light housekeeping related to patient care
  • Companionship and emotional support
  • Reporting changes in patient condition to the supervising nurse

Who Qualifies

  • Patients receiving skilled nursing or therapy who need personal care support
  • Patients with limited mobility or self-care ability
  • Patients whose family caregivers need respite or supplemental support
  • Note: HHA services must be ordered alongside a skilled service (nursing or therapy)

Visit Frequency

Available daily or several times per week as ordered by the physician

Medicare Coverage

Covered by Medicare Part A when ordered alongside a skilled service and homebound criteria are met.

Home Health Aide services cannot be ordered as a standalone service — they must accompany skilled nursing or therapy.
